Cypress Company Cited In Worker’s Death

Construction & Refinery Accidents

A Cypress company has been cited by federal officials after a worker died in a trench collapse while repairing a broken water line last month.

Municipal District Services LLC, which provides wastewater treatment and water construction services, failed to provide cave-in protection for workers in a trench, according to the Occupational Safety and Health Administration. OSHA cited the company for a willful violation with a fine of $63,000.

The company excavated a trench through a concrete road, and the men entered the trench to clean up and cut a broken pipe, according to OSHA. The south wall of the trench, which was about 16 feet long and 8 to 10 feet deep, collapsed. One man was killed and another injured in the Dec. 5 incident.

Municipal District Services is continuing its investigation into the incident, a company spokeswoman said. It has 15 business days to either pay the penalty, contest the findings or request an informal conference with the federal agency.

“We strongly disagree with the findings of the governmental agency that our conduct was willful,” said company vice president Rebecca Marcucci.
